VRd (Velcade/Revlimid/Dex): what to expect, day by day

A three-drug combination for myeloma. The effect to watch most is nerve tingling in the hands/feet from bortezomib — report it early, as newer weekly under-the-skin dosing keeps it much lower. You'll take a blood thinner (often aspirin) to prevent clots, plus an antiviral to prevent shingles. Lenalidomide requires strict pregnancy prevention.

Bortezomib (injection, now usually WEEKLY and SUBCUTANEOUS) + lenalidomide (oral days 1-14) + dexamethasone (oral, specific days), in 21-day cycles. Bortezomib's nerve effect is CUMULATIVE (much lower with weekly/under-the-skin dosing). A blood-clot risk runs throughout, so a blood thinner (often aspirin) is given. Lenalidomide requires strict pregnancy prevention.

Get help straight away — this is true on any regimen, on any day.

A temperature of 100.4°F (38°C) or higher during treatment can mean a serious infection while your blood counts are low. Call your team or go to the ER — do not wait it out.

Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat · new confusion or fainting · shaking chills · not being able to keep fluids down.

Neutropenia

Typically around days 11–21

A drop in infection-fighting white cells from lenalidomide. Your team checks counts and may adjust the dose or add a booster.

Call your team urgently: A temperature of 100.4°F (38.0°C) or higher — an emergency.

Low platelets / anemia

Typically around days 11–21

Drops in platelets (bruising/bleeding) and red cells (tiredness). Bortezomib's platelet dip is typically cyclical, recovering between cycles.

Call your team: Bleeding that won't stop, black/tarry stools, or new breathlessness.

Not tied to a cycle day

These build with the number of doses, or are measured from when treatment started, so a cycle day would be the wrong shape for them. Each keeps the timing its own source states.

Peripheral neuropathy

builds with dosing; can appear/worsen any time; may be permanent

Bortezomib's signature effect: numbness, tingling, or burning pain in the hands and feet. It builds with dosing and can become long-lasting or permanent, so tell your team at the first sign — dose changes help. Modern WEEKLY, under-the-skin dosing makes severe neuropathy much less likely (grade 3+ roughly 6% under-the-skin vs 15% into-the-vein).

Call your team: Numbness or pain that makes it hard to button clothes, walk, or feel the floor — report early.

Blood clots (DVT / PE)

any time; elevated throughout treatment

Lenalidomide (with dexamethasone and myeloma itself) raises the risk of clots in the legs or lungs — so you'll take a blood thinner, usually aspirin, to prevent them. Report leg swelling/pain or sudden breathlessness or chest pain at once.

Call your team urgently: Sudden breathlessness, chest pain, or a swollen, painful calf — emergency.

Shingles (herpes zoster) reactivation

any time on treatment

Bortezomib raises the risk of a shingles flare (a painful blistering rash in a band), so you'll take a preventive antiviral, which greatly lowers the risk. Report any painful blistering rash early.

Call your team urgently: A painful blistering rash, especially near the eye — seek care.

Diarrhea

Common loose stools. Late-onset lenalidomide diarrhea sometimes responds to a specific bile-acid medicine — mention it if it persists.

Steroid effects (dexamethasone)PROVISIONAL

On steroid days you may notice higher blood sugar, trouble sleeping, mood changes, and stomach irritation, plus added infection risk. Take steroids in the morning if advised, monitor glucose if diabetic, and don't stop them suddenly.

Pregnancy risk — strict prevention required (lenalidomide)

Lenalidomide causes severe birth defects and must never be taken during pregnancy. A strict prevention program applies: pregnancy testing and two reliable birth-control methods (or abstinence) during and for 4 weeks after treatment, for anyone who could become pregnant.
