VAC (Ewing Sarcoma): what to expect, day by day

A strong, curative-intent chemo combination for Ewing sarcoma. Expect very low blood counts (usually with a booster), hair loss, and nausea. Vincristine can cause nerve tingling and constipation (use the bowel routine), and extra fluids/mesna protect the bladder from cyclophosphamide — report any blood in the urine. Heart function is monitored because of doxorubicin.

Vincristine + doxorubicin + cyclophosphamide (often alternating with other drugs) — an intensive, curative-intent regimen for Ewing sarcoma. Blood-count drops are nadir-based (~days 7-14) and often deep, with a white-cell booster. Cyclophosphamide is given with mesna/fluids to protect the bladder. Vincristine's nerve effects and doxorubicin's heart effect are CUMULATIVE.

Get help straight away — this is true on any regimen, on any day.

A temperature of 100.4°F (38°C) or higher during treatment can mean a serious infection while your blood counts are low. Call your team or go to the ER — do not wait it out.

Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat · new confusion or fainting · shaking chills · not being able to keep fluids down.

Timed to the cycle

Neutropenia and febrile neutropenia

Typically around days 8–21

Deep drops in infection-fighting white cells, often with fever needing hospital antibiotics. A white-cell booster is usually given; any fever is treated urgently.

Call your team urgently: Any fever of 100.4°F (38.0°C) or higher — an emergency.

Not tied to a cycle day

These build with the number of doses, or are measured from when treatment started, so a cycle day would be the wrong shape for them. Each keeps the timing its own source states.

Nerve effects and constipation (vincristine)

builds over doses; often improves after treatment

Vincristine can cause numbness, tingling, or weakness and commonly slows the bowel (constipation, occasionally a blockage). Use the recommended bowel routine and report constipation with belly pain or new weakness.

Call your team: No bowel movement for 3 days with belly pain, or new weakness/trouble walking.

Bladder irritation (hemorrhagic cystitis)

Cyclophosphamide can irritate the bladder and cause bleeding, so mesna and extra fluids are given to protect it. Report burning with urination or any blood in the urine.

Call your team: Blood in the urine, or trouble passing urine.

Heart-muscle weakening (doxorubicin, cumulative)

cumulative; can appear months-years later

Doxorubicin can weaken the heart's pumping, with risk rising as the total dose adds up — so the cumulative dose is tracked and heart function is checked (echo). Report breathlessness or swelling.

Call your team: New shortness of breath, swelling, or a rapid/irregular heartbeat.

Nausea and hair loss

Sickness in the first 24 hours is the "acute" phase and usually peaks 2-3 hours in; anything from 24 to 120 hours is the "delayed" phase.

Nausea is common (managed with anti-nausea medicine) and hair loss is near-universal, growing back after treatment ends.

Small long-term risk: secondary leukemia

years after treatment

A small long-term risk of a treatment-related blood cancer years later, from the chemotherapy. It's uncommon and outweighed by the benefit of curing the cancer, but it's why long-term follow-up matters.
