This is hormone replacement, not chemotherapy — a daily pill that replaces the thyroid hormone your body no longer makes. When the dose is right, you should feel normal with no side effects. Any symptoms usually mean the dose needs a small adjustment: too much causes a racing heart, tremor, or trouble sleeping; too little causes tiredness, feeling cold, or weight gain. Blood tests guide the dose.

This is NOT chemotherapy. After the thyroid is removed, levothyroxine simply REPLACES the thyroid hormone your body used to make — a daily pill, usually for life. In thyroid cancer it's often dosed slightly high on purpose to keep the hormone TSH low (which discourages any cancer regrowth). When the dose is right, there are essentially no side effects; symptoms come only from the dose being too high or too low, and are fixed by adjusting it based on blood tests.

Get help straight away — this is true on any regimen, on any day.

A temperature of 100.4°F (38°C) or higher during treatment can mean a serious infection while your blood counts are low. Call your team or go to the ER — do not wait it out.

Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat · new confusion or fainting · shaking chills · not being able to keep fluids down.

Too much thyroid hormone causes symptoms like a fast or pounding heartbeat, tremor, feeling hot/sweaty, anxiety, trouble sleeping, and unintended weight loss. Long-term (with intentional TSH suppression)

When the dose is kept deliberately on the higher side for years to suppress TSH (to lower cancer-regrowth risk), there can be small long-term effects on the bones (thinning) and heart rhythm.
