Immunotherapy for advanced skin cancer that helps your own immune system fight it. Most people tolerate it well, but the immune system can occasionally inflame organs (skin, bowel, lungs, liver, hormone glands). Reporting new symptoms early lets your team treat these quickly; the most common everyday effects are tiredness, rash, and diarrhea.

Cemiplimab is immunotherapy given every 3 weeks (for advanced non-melanoma skin cancers). Like other checkpoint drugs, its main effects are IMMUNE-RELATED — the immune system can inflame almost any organ, at ANY time, even after stopping. Onset is measured in weeks-months, not infusion days.

Get help straight away — this is true on any regimen, on any day.

A temperature of 100.4°F (38°C) or higher during treatment can mean a serious infection while your blood counts are low. Call your team or go to the ER — do not wait it out.

Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat · new confusion or fainting · shaking chills · not being able to keep fluids down.

Fatigue

Tiredness is the most common effect. If it's marked, your team may check for a hormone-gland (thyroid/adrenal) cause.

Rash / itch

Rash and itching are common, usually mild. Rarely, a serious blistering/peeling reaction occurs — report a spreading rash, blisters, or peeling, especially with mouth/eye sores.

The immune system can inflame the bowel, causing diarrhea or cramping. Report new or worsening diarrhea early — untreated it can become serious; it's treated with steroids.

Call your team urgently: Diarrhea with blood/mucus, severe cramping, or many extra stools a day — call urgently.

Immune-related pneumonitis / hepatitis

Uncommonly, the immune system inflames the lungs (new cough/breathlessness) or the liver (rising liver tests, yellowing). Report breathing changes or yellowing of skin/eyes promptly; these are treated with steroids.

Hormone-gland problems (thyroid, adrenal)

The immune system can affect hormone glands — most often the thyroid (under- or over-active), occasionally the adrenal or pituitary. Some effects are permanent and need hormone replacement. Report marked tiredness, dizziness, or temperature/weight changes.
