A chemo-plus-immunotherapy combination for small cell lung cancer. The chemo lowers blood counts significantly (with infection risk), causes hair loss and nausea; durvalumab can occasionally cause immune-related inflammation, most often of the thyroid.
Chemo (carboplatin + etoposide) plus immunotherapy (durvalumab) for extensive-stage small cell lung cancer. The chemo backbone is heavily myelosuppressive (nadir-based, days since infusion); durvalumab's immune effects can occur any time (weeks-months).

A temperature of 100.4°F (38°C) or higher during treatment can mean a
serious infection while your blood counts are low. Call your team or go to the ER — do not
wait it out.

Neutropenia
Typically around days 7–28
A drop in infection-fighting white cells — often significant with this chemo; the low point is usually around day 10-14. A white-cell booster is frequently used.
Call your team urgently: A temperature of 100.4°F (38.0°C) or higher — an emergency.

Febrile neutropenia
Typically around days 7–28
Fever occurring while white cells are low — a medical emergency needing immediate antibiotics.
Call your team urgently: Fever with feeling unwell — go to the ER and say you're on chemotherapy.

Anemia
Typically around days 7–28
A drop in red cells causing tiredness and breathlessness, building over cycles; transfusion support is sometimes needed.
Call your team: New or worsening breathlessness, chest pain, or a racing heart.

Thrombocytopenia
Typically around days 7–28
A drop in platelets (which help blood clot); the low point is usually around day 14-21. Watch for easy bruising or bleeding.

Not tied to a cycle day

Hair loss
begins ~2-4 weeks in

Nausea
Sickness in the first 24 hours is the "acute" phase and usually peaks 2-3 hours in; anything from 24 to 120 hours is the "delayed" phase.
Usually mild-moderate and well controlled with anti-nausea medicine; can be delayed a few days.
Call your team: Can't keep fluids down for more than 12 hours.

Fatigue
Tiredness that builds over cycles. If marked, your team may check thyroid function (an immune effect of durvalumab).

Immune-related effects (thyroid, lungs, liver)

From the immunotherapy: the immune system can inflame the thyroid (most common), or less often the lungs, liver, or bowel — at any time. Report new cough/breathlessness, marked tiredness, yellowing, or diarrhea; these respond well to early treatment.
Call your team urgently: Severe breathlessness, diarrhea, or feeling very unwell — contact your team urgently.
