A curative but intensive 3-drug regimen for testicular/germ-cell cancer. The most important thing to watch is bleomycin's effect on the lungs (report any new breathlessness or cough), alongside lowered blood counts, strong nausea, and cumulative kidney/hearing/nerve effects from cisplatin.
Bleomycin + etoposide + cisplatin in 21-day cycles (etoposide+cisplatin days 1-5, bleomycin weekly). Blood-count drops are nadir-based (days since infusion). Bleomycin's lung toxicity and cisplatin's kidney/hearing/nerve effects are CUMULATIVE — they build with total dose, not a single day.
Get help straight away — this is true on any regimen, on any day.
A temperature of 100.4°F (38°C) or higher during treatment can mean a
serious infection while your blood counts are low. Call your team or go to the ER — do not
wait it out.
Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat ·
new confusion or fainting · shaking chills · not being able to keep fluids down.
This is standing guidance shown to everyone. Nothing on this page is based on anything
about you, and nobody is monitoring you.
2 of the 9 entries below are marked provisional.
They are shown because leaving them out would be its own distortion, but they have not yet
been checked against the source document by a person, and they are not presented as
authoritative. Every provisional entry is labelled where it appears.
Timed to the cycle
Day 1 is the day of your infusion. These are the effects a retrievable
document ties to a day range for this regimen — not a prediction about you.
Fever and chills (bleomycin)PROVISIONAL
During or just after an infusion
Bleomycin can cause fever and chills within hours of a dose, especially early on. It's usually self-limited; your team may pre-medicate. Tell them so infection can be ruled out.
reported by about ~20-60% of people · Timing is part of what this effect is · from Bleomycin
Neutropenia
Typically around days 7–39
A temporary drop in infection-fighting white cells; the low point is usually 1-2 weeks after the day-1-to-5 infusions. Fever while counts are low is an emergency.
Call your team urgently: A temperature of 100.4°F (38.0°C) or higher — an emergency.
reported by about 60-91% of people; severe in 3-17% · Timing from a drug-monograph reference · from Etoposide, Cisplatin
Febrile neutropenia
Typically around days 7–39
Fever occurring while white cells are low — most often in the first cycle. It's a medical emergency needing immediate antibiotics.
Call your team urgently: Fever with feeling unwell — go to the ER and say you're on chemotherapy.
reported by about 17% of people · Timing from a drug-monograph reference · from Etoposide, Cisplatin
Not tied to a cycle day
These build with the number of doses, or are measured from when treatment started, so a cycle day would be the wrong shape for them. Each keeps the timing its own source states.
cumulative, dose-related (risk rises past ~300 units); can develop over/after the course
Bleomycin's signature risk: inflammation of the lungs that can progress to scarring (fibrosis). It's dose-related — the risk climbs with total bleomycin given (notably past ~300 units) and with older age or reduced kidney function. Report ANY new or worsening breathlessness, dry cough, or chest discomfort promptly — it can be serious and is not always reversible.
Call your team urgently: New or worsening breathlessness or cough — contact your team urgently.
reported by about 7-10% of people; severe in ~1% fatal · from Bleomycin
Nausea and vomiting
Sickness in the first 24 hours is the "acute" phase and usually peaks 2-3 hours in; anything from 24 to 120 hours is the "delayed" phase.
Cisplatin is strongly nausea-causing, so you'll get robust anti-nausea prevention. Nausea can be delayed, lasting several days after the day-1-to-5 doses.
Call your team: Can't keep fluids down for more than 12 hours.
reported by about ~90% of people; severe in 5-15% · Timing from a clinical guideline · from Cisplatin
Kidney injury (nephrotoxicity)PROVISIONAL
cumulative, dose-dependent over cycles
Cisplatin can stress the kidneys, so you'll receive generous IV fluids around each dose. It can also lower magnesium and other salts. Your team checks kidney bloodwork each cycle; the effect can build over cycles.
Call your team: Passing much less urine than usual, or marked swelling.
reported by about ~20-30% of people · from Cisplatin
Hearing changes (ototoxicity)
cumulative, dose-dependent (high-frequency)
Cisplatin can cause ringing in the ears or high-pitch hearing loss that builds with cumulative dose and may be permanent. Report any new ringing or hearing change promptly.
reported by about ~10-31% of people · from Cisplatin
Hair loss
over the first weeks
Hair loss is common with this regimen and grows back after treatment ends.
reported by about ~60-90% of people · from Etoposide
Skin changes (bleomycin)
builds over repeated doses
Bleomycin commonly causes skin changes — darkening (hyperpigmentation), thickening, tenderness, or (uncommonly) cold-triggered color changes in the fingers. Most improve after treatment ends.
reported by about ~50% of people · from Bleomycin

Definitional — an infusion reaction is one that occurs during or shortly after the infusion — “The timing is part of what the term denotes; there is no empirical claim here to source.”
A window appears here only where a retrievable document ties it to a drug in
this regimen. Where nothing could be found, the window was removed rather than estimated —
so a missing time is an honest gap, not an oversight.
