A newer daily targeted pill for CML, generally better tolerated than the older options. The main things to monitor are blood counts, the pancreas enzyme lipase, and blood pressure. It largely avoids the puffiness/cramps, lung fluid, and heart-rhythm issues of older CML drugs, though a small blood-vessel risk still applies.
Oral asciminib taken daily, continuously (a newer 'STAMP' targeted inhibitor). It's generally BETTER TOLERATED than older CML pills — largely avoiding imatinib's swelling/cramps, dasatinib's lung fluid, and nilotinib's boxed heart-rhythm/metabolic burden. Still, it can lower blood counts, raise the pancreas enzyme lipase, and raise blood pressure. (Note: the headline incidence figures come from heavily pre-treated patients, so they may look higher than in first-line use.)
Get help straight away — this is true on any regimen, on any day.
A temperature of 100.4°F (38°C) or higher during treatment can mean a
serious infection while your blood counts are low. Call your team or go to the ER — do not
wait it out.
Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat ·
new confusion or fainting · shaking chills · not being able to keep fluids down.

They are shown because leaving them out would be its own distortion, but they have not yet

What is reported
This regimen is taken continuously, so effects are described in the terms their sources use rather than by cycle day.
Generally well tolerated
A key point: asciminib's newer mechanism means most people have milder side effects than with older CML pills — it largely avoids the facial puffiness/cramps of imatinib, the lung fluid of dasatinib, and the heart-rhythm/metabolic burden of nilotinib. The effects below are the ones still worth monitoring.
reported by about milder off-target profile% of people · from Asciminib
Low platelets (thrombocytopenia)
first months; usually recovers
A drop in platelets, mostly in the first months (rates look higher partly because studied patients were heavily pre-treated). Watch for easy bruising or bleeding; counts are monitored.
Call your team: Bleeding that won't stop, or black/tarry stools.
reported by about 46% of people; severe in 24% · from Asciminib
Neutropenia
first months; usually recovers
A drop in infection-fighting white cells, mostly early, then usually recovering. Bloodwork is checked (about every 2 weeks for the first 3 months).
Call your team urgently: A temperature of 100.4°F (38.0°C) or higher — an emergency.
reported by about 39% of people; severe in 17% · from Asciminib
Raised lipase / pancreas irritation
any time on treatment
Asciminib can raise the pancreas enzyme lipase and, uncommonly, cause pancreatitis (~2-3%): severe upper-belly pain radiating to the back with nausea. Your team monitors lipase; report severe belly pain.
Call your team urgently: Severe upper-belly pain radiating to the back with vomiting.
reported by about 14% (pancreatitis ~2.5%) of people; severe in 3.9% · from Asciminib
High blood pressure (hypertension)
weeks-months
Blood pressure can rise on treatment. Your team checks it and treats it with medicine if needed.
Call your team: Severe headache, vision changes, or chest pain.
reported by about 14% of people; severe in 6% · from Asciminib
Muscle and joint pain
Aching muscles or joints, usually manageable with simple measures.
reported by about 22% of people; severe in 2.6% · from Asciminib
Headache
Common and usually mild; simple pain relief helps.
reported by about 19% of people; severe in 1.9% · from Asciminib
Blood-vessel (arterial) events — lower but realPROVISIONAL
accrues over time; may be permanent
Like other CML pills, asciminib carries a small risk of artery-narrowing events (heart, brain, limbs), lower than with nilotinib but still real. Your team manages cardiovascular risk factors; report chest pain, stroke symptoms, or severe leg pain.
Call your team urgently: Chest pain, sudden weakness/speech trouble, or severe leg pain/coldness — emergency.
reported by about ~3-5% of people · from Asciminib
